"But My Scans Are Clear": Why Pain Persists When Everything Looks Normal

There is almost nothing more frustrating than sitting in a doctor's office, in real, exhausting pain, only to hear the words: "Your X-rays are completely clear. Everything looks normal."
It feels invalidating. It might even make you feel like people think you're making it up. But here is the most important thing you need to know: your pain is 100% real, even if a scan can't see it.
To understand why pain sticks around long after an injury has healed—or even when there was never a clear injury to begin with—we have to stop looking at the muscles and joints, and look instead at the body's ultimate control center.
Sensory signals travel from the body, up the spinal cord, and into the brain to be processed. In chronic pain, these specific pathways become hyperactive.
The Broken Alarm System
Think of acute (short-term) pain like a highly effective fire alarm. The alarm goes off when the burglar enters your house, so he can't steal anything. If you touch a hot stove, the nerve endings in your hand instantly send a danger signal up to your brain. Your brain registers "PAIN!" and you yank your hand away. The system protected you.
But sometimes, that alarm system malfunctions. Chronic pain is what happens when the fire has been put out (the tissues have fully healed), but the alarm keeps blaring anyway. Now instead of going off when a burglar breaks in, it goes off when the wind blows the curtain or a butterfly flies past. Your nervous system has essentially gotten stuck in the "on" position.
The Science: Central Sensitisation
We call this phenomenon central sensitisation.
Normally, your nervous system filters out harmless sensations—like the feeling of a shirt against your skin or the normal, everyday stretch of a muscle. But when the central nervous system becomes sensitised, it’s like someone cranked the volume dial on an amplifier all the way up to 10. The nerves become so excitable and hyper-reactive that they start interpreting completely safe, normal movements as dangerous.
The takeaway: The pain you feel isn't a sign of ongoing tissue damage; it's a sign of a highly overprotective nervous system.
This happens because of neuroplasticity—the brain's ability to adapt and rewire itself. If you experience pain for a long time, your nervous system actually gets more efficient at producing danger signals. It quite literally learns the habit of being in pain.
The "Amplifier" Effect
It’s also crucial to understand that pain isn't just a simple input/output equation. The brain processes physical pain in the exact same neighborhoods that process emotions, stress, and sleep.
When you are exhausted, anxious, or highly stressed, your brain is already in a state of high alert. This lowers your physical pain threshold, amplifying the signals. Stress doesn't invent the pain out of thin air, but it does act like a megaphone for the signals already traveling through your body.
The Good News
If your pain has persisted despite "normal" test results, it doesn't mean your body is broken beyond repair.
Because neuroplasticity works in both directions, a sensitised nervous system can be desensitised. Modern treatments for chronic pain often focus less on "fixing" the body part that hurts, and more on retraining the brain. Through specialized physical therapy, gentle movement exposure, and stress management, you can slowly teach your nervous system that you are safe and finally turn the alarm off.
